1. THE MAN FIRST. Call 911 if there is any doubt at all. One
   person stays with him. Nobody says a word about cost.
2. STOP THE JOB. Clear that area. Nothing gets cleaned up yet.
3. CALLS IN ORDER: foreman -> office -> owner -> his family.
4. PHOTOS BEFORE CLEANUP. Wide, then close: the ground, the
   ladder, the anchor, the harness, the deck, the tools.
5. WHO SAW IT: ______________________________________________
   ____________________________________________________________
6. WHAT HAPPENED, in his words and theirs. Written down TODAY.
7. OSHA CLOCK. A death, an in-patient hospitalization, an
   amputation, or a lost eye must be reported, and the
   deadlines are not the same. Read the OSHA card. Do not guess.
8. CLAIM OPENED SAME DAY.
